Advice:
A dedicated admin reports team deal with any requests that are handed into the practice. If you have a medical report or form that needs to be completed please hand these into the reception and someone from the reports team will be in contact with you if there is a fee to be paid before completion.
We ask that you allow up to 28 days for the process to be completed.
